Accepts Insurance: I accept Blue Cross Blue Shield PPO plans. I am happy to provide necessary documentation to file with other insurance carriers for reimbursement.

** All regular scheduled therapy sessions are 55-minutes long

I will gladly provide you with a superbill (forms used by insurance for billing) for you to submit to your insurance company if they offer out of network benefits. Please note that insurance companies will require a diagnosis on the superbill. This diagnosis goes in your medical record. This may be perfectly fine for you. However, if you are not ok with this, then paying out of pocket is the best way to maintain your privacy.
If you want to find out about your benefits, call the customer service number on the back of your insurance card and ask to speak to someone about out-of-network mental health benefits. They’ll talk to you about your deductible and coverage.
Under the Texas Health and Safety Code Section 181.105 (effective 9/1/25), I am required to provide patients with these instructions about your right to request your health records, contact my disciplinary and licensing board, and file a consumer complaint.
How to Request Your Health Care Records:

You have the right to request your health care records from me. Active patients may request their records via their secure portal messaging access. Former patients may request their records by emailing me at drdlm@realigncounselingservices.com

Please note that while you have the legal right to request health care records, clinical psychotherapy notes are written as technical medical documents to fulfill insurance requirements, track treatment progress, and coordinate care. They contain clinical language that can be difficult to interpret and potentially distressing. They are not intended to demonstrate the full depth of our relational work together. As such, I strongly recommend I provide you with a treatment summary instead of psychotherapy notes and strongly recommend that we meet to discuss the contents of any records I provide you. Please note that fees applicable to my time to create the summary and our meeting about the contents of any clinical documentation I provide you are generally not covered by insurance and may be paid by credit or debit card.
